The Patient

Wound Care Anywhere LLC provides chronic wound care services for a patient in a variety of settings, including a patient’s home (also a patient’s family member or friends home), nursing home and rehabilitation center, assisted living facility, or any of the Lister Healthcare Clinics.

Facility Support

Wound Care Anywhere provides acute and chronic wound care services in nursing facilities and assisted living facilities. We work directly with the facilities staff on a weekly basis to create a treatment plan specific to the patient. This is a free service to the facility.

Referrals

Wound Care Anywhere accepts referrals for patients with chronic wound/s from:

  • A patient’s family member
  • A patient’s friend
  • Nursing home and rehabilitation centers
  • Assisted living facilities
  • Home health
  • Hospitals
  • Primary care clinics
  • Specialty groups
